Analgesic Effect of Local Wound Infiltration with Magnesium Sulfate Versus Bupivacaine in Reducing Postoperative Pain in Patients Undergoing Inguinal Hernia Repair Surgery: A Randomized, Double-blind, Controlled Study

المقدمة يعد ألم ما بعد الجراحة من أكثر المشاكل شيوعًا بعد عملية إصلاح الفتق الإربي. فالحد من الألم بعد الجراحة يؤدي إلى تسريع التعافي الوظيفي، ويقلل من مدة الإقامة في المستشفى والمراضة بعد الجراحة. يوصى بترشيح الجرح باستخدام التخدير الموضعي لتقليل استهلاك المواد الأفيونية قبل الجراحة وآلام ما بعد الجراحة. هدفت هذه الدراسة إلى تقييم فعالية البوبيفاكايين مقابل كبريتات المغنيسيوم في جراحات إصلاح الفتق الإربي وذلك بحقن الجرح قبل إغلاقه، لتقليل حدوث وشدة الألم بعد العملية الجراحية، واستهلاك المسكنات وزيادة وقت طلب الجرعة الأولى المسكنة. الأهداف الهدف من هذه الدراسة هو تحديد التأثير المسكن لترشيح كبريتات المغنيسيوم مقابل البوبيفاكايين قبل إغلاق العملية الجراحية لإصلاح الفتق الاربي في تقليل حدوث وشدة الألم بعد الجراحة واستخدام المواد الأفيونية، لتقليل استهلاك المسكنات وزيادة وقت الجرعة الاولى للمسكنات. تصميم الدراسة في تجربة سريرية مزدوجة عمياء، تم تحديد المرضى المخطط لهم عملية إصلاح الفتق الإربي الاختيارية. بعد موافقة مجلس المراجعة المؤسسية والتوقيع على استمارات الموافقة على المشاركة في الدراسة، يتم تقديم 80 مريضًا، من الحالة الجسدية الأولى والثانية حسب الجمعية الامريكية لأطباء التخدير، تتراوح أعمارهم بين 18 و70 عامًا، لإصلاح الفتق الإربي تحت التخدير العام الناتج عن إعطاء الفنتانيل، (2 ميكروغرام / كغ)، بروبوفول (2 ملغم / كغم) وروكورونيوم (0.6 ملغم / كغم) ويتم الحفاظ عليه بنسبة 70٪ / 30٪ من أكسيد النيتروز والأكسجين والأيزوفلورين بتركيز 0.5٪ إلى 1.5٪ .بعد اجراء عملية التخدير، قسم المرضى عشوائياً إلى مجموعتين وقبل إغلاق الشق، تم ارشاح الجرح في مجموعة البوبيفاكائين ب 5 مل من بوبيفاكين 0.5٪ يضاف إلى 5 مل من محلول ملحي عادي يتم حقنها تحت الجلد وفي مجموعة المغنيسيوم 10 مل سلفات مغنيسيوم 20٪ تحقن تحت الجلد. تم استخدام المورفين 2 ملغ في الوريد كدواء إنقاذ. تم قياس قيم الألم باستخدام المقياس التناظري البصري (VAS) عند 0.5، 1، 2، 3، 6، 12، 18 و24 ساعة بعد الشفاء من التخدير. تم توثيق إجمالي استهلاك المورفين ووقت الطلب الأول لجرعة المورفين والآثار الجانبية لأدوية الدراسة. النتائج في هذه الدراسة، قارنا ارتشاح الجرح بحقن المغنيسيوم والبوبيفاكائين في موقع جراحة الفتق الإربي قبل الإغلاق للسيطرة على الألم بعد الجراحة. أظهرت نتائجنا تأثيرًا متفوقًا من مادة بوبيفاكايين مقارنةً بالمغنيسيوم في تقليل الألم وتقليل الأدوية المسكنة وزيادة وقت طلب الجرعة الأولى من المسكنات خلال 24 ساعة بعد العملية. في هذه الدراسة، تم التحقق من تجانس المشاركين عن طريق اختبار ليفين، وكانت العينة متجانسة في العمر والوزن والطول (قيمة P> 0.005) مما يعني عدم وجود اختلاف كبير بين مجموعتين في البيانات الديموغرافية. فيما يتعلق بمقياس التناظر البصري خلال الفترة الإجمالية البالغة 24 ساعة بعد الجراحة التي تعكس شدة الألم بعد الجراحة، أظهرت النتائج أن مقياس التناظر البصري في مجموعة المغنيسيوم (mean (SD) (1.88 ± 0.53) أعلى بكثير من مقياس التناظر البصري في مجموعة البوبيفاكائين (mean (SD) = 0.98 ± 0.61)، وتشير النتائج إلى أن البوبيفاكائين أكثر فعالية في الحد من آلام ما بعد الجراحة من كبريتات المغنيسيوم. أظهرت النتائج وجود فروق ذات دلالة إحصائية في نسبة حدوث الألم خلال الفترة الكلية البالغة 24 ساعة. بعد الجراحة بين المجموعتين التجريبيتين (P-value = 0.000 <0.05)، أظهرت النتائج أن عدد المرضى الذين يعانون من الألم في مجموعة كبريتات المغنيسيوم (n (%) = 38 (95%)) أعلى بكثير من مجموعة البوبيفاكائين (P = 0.000), (n (%) = 18 (45%)) تشير النتائج إلى أن البوبيفاكائين أكثر فعالية في الحد من حدوث آلام ما بعد الجراحة من كبريتات المغنيسيوم. فيما يتعلق بطلب المورفين خلال الفترة الإجمالية البالغة 24 ساعة بعد الجراحة، أظهرت النتائج أن عدد المرضى الذين تناولوا المورفين (2 mg i.v.) في المجموعة كبريتات المغنيسيوم (n (%) = 38 (95%))) أعلى بكثير من مجموعة البوبيفاكين. (n (%) = 17 (42.5%)، (P = 0.000) النتيجة تشير إلى أن مجموعة البوبيفاكايين أكثر فعالية في تقليل استهلاك المواد الأفيونية من عقار المورفين من مجموعة كبريتات المغنيسيوم. توجد فروق ذات دلالة إحصائية في الاستجابة الكاملة (لا غثيان ولا قيء ولا دواء إنقاذ) بين المجموعتين التجريبيتين (قيمة P = 0.004 <0.05)، وأظهرت النتائج عدد المرضى الذين لديهم استجابة كاملة في مجموعة كبريتات المغنيسيوم (n (%) = 14 (35%)) أقل بكثير من مجموعة بوبيفاكائين (n (%) = 27 (67.5%))،(P = 0.004) . تشير النتائج إلى أن عدد المرضى الذين يعانون من استجابة كاملة في مجموعة بوبيفاكايين أكثر من مجموعة كبريتات المغنيسيوم. كما توجد فروق ذات دلالة إحصائية في الغثيان بين المجموعتين التجريبيتين (P value = 0.003 <0.05) ، وأظهرت النتائج أن عدد المرضى الذين عانوا من الغثيان في مجموعة كبريتات المغنيسيوم (n (%) = 22 (55%)) أعلى بكثير من واحد في مجموعة البوبيفاكائين (n (%) = 9 (22.5%)). وأظهرت النتائج عدم وجود فروق ذات دلالة إحصائية في القيء بين المجموعتين التجريبيتين (P-value = 1,000> 0.05) . توجد فروق ذات دلالة إحصائية في ضغط الدم بعد مرور 4 ساعات (S less 120 and D less 80) بين المجموعتين التجريبيتين،(p-value of the post hoc Multiple comparison test <0.05) ، وعدد المرضى الذين 0لديهم ضغط دم (S less 120 and D less 80) in في مجموعة كبريتات المغنيسيوم(N(%)=8(20%)) أقل بكثير من مجموعة البوبيفاكين (N(%)=17(42.5%)).. تشير النتيجة إلى أن المزيد من المرضى في مجموعة البوبيفاكين كان لديهم ضغط دم أقل من 120/80 بعد 4 ساعات. توجد فروق ذات دلالة إحصائية في ضغط الدم بعد 16 ساعة. (S less than 120 and D less 80) بين المجموعتين التجريبيتين (P-value of the post hoc Multiple comparison test <0.05) ، عدد المرضى الذين لديهم ضغط دم بعد 16 ساعة (S less than 120 and D less 80) in MgSO4 لا group(N(%)=16(40%)) أقل بكثير من مجموعة (N(%)=27(67.5%)). تشير النتيجة إلى أن المزيد من المرضى في مجموعة البوبيفاكين لديهم ضغط دم أقل من 120 / 80 بعد 16 ساعة. توجد فروق ذات دلالة إحصائية في ضغط الدم بعد مرور 8 ساعات (Stage 2 - S 140 or higher and D 90 or higher) بين المجموعتين التجريبيتين (P-value of the post hoc Multiple comparison test <0.05)، المرضى الذين لديهم ضغط دم بعد مرور 8 ساعات (Stage 2 - S 140 or higher and D 90 or higher) في مجموعة كبريتات المغنيسيوم (N(%)=10(25%)) أعلى بكثير من مجموعة البوبيفاكين (N(%)=3(7.5%)). تشير النتيجة إلى أن المزيد من المرضى في مجموعة كبريتات المغنيسيوم لديهم BP≥ 140/90. يمكن أن يكون نتيجة للألم. في الختام، تشير النتيجة إلى أن المزيد من المرضى في مجموعة البوبيفاكين لديهم ضغط دم أقل من 120/80 من مجموعة كبريتات المغنيسيوم (P-value=0.043<0.05). توجد فروق ذات دلالة إحصائية في معدل التنفس الكلي بين المجموعتين ، وأظهرت النتائج أن معدل التنفس في مجموعة كبريتات المغنيسيوم (mean=16.88) أعلى بكثير من معدل التنفس في مجموعة البوبيفاكايين(mean=16.59) . لكن الفرق ليس مهمًا من الناحية السريرية. الاستنتاج كان ترشيح بوبيفاكائين للجروح في موقع الجراحة أكثر فعالية من ترشيح كبريتات المغنيسيوم لتقليل حدوث الألم ما بعد الجراحة وشدته، وتقليل استهلاك المواد الأفيونية، وزيادة الوقت الأولي لطلب مسكنات الآلام بعد الجراحة في المرضى الذين يخضعون لجراحة الفتق الإربي. توصية في الدراسات المستقبلية، تناول المغنيسيوم كعامل مساعد وقياس مستويات المغنيسيوم في الدم قبل الجراحة وبعدها لتحقيق نتيجة مهمة. بناءً على النتائج، اتضح أن استخدام ترشيح بوبيفاكائين في إصلاح الفتق الإربي أكثر فعالية من كبريتات المغنيسيوم ويوصى باستخدام هذا الدواء لتقليل الألم بعد الجراحة والآثار الجانبية الطفيفة.
Clinical Comparison Between Isoflurane and Sevoflurane In Patients with Open-Heart Surgery: A Prospective, Observational Study from Palestine

Background: volatile anesthetic agents sevoflurane and isoflurane gases can effects on hemodynamic intra- and post-operative course on-pump coronary artery bypass surgery in patients. Anesthesia type affects adequate depth of anesthesia, reduction in need of analgesic dosage, early extubation and transfer from the Intensive Care Unit (ICU). Objectives: Study objectives were to compare the induction and recovery criteria following anesthesia with sevoflurane and isoflurane in open heart surgery, intra and post operation. Methods: This study was an observational, cohort prospective study, it was performed at the Arab Specialized Hospital in Nablus, Heart Center in the West Bank/Palestine between May – August 2016 on all patients who were between 30-70 years and admitted to open heart surgery. All information was collected from files and observational and data collection. The statistical package of social science (SPSS) was used for data entry in statistical analysis. Results: During the study period, 63 patients were included, among them, 32 patients were given isoflurane, and 31 patients were given sevoflurane during open heart surgery. Systolic blood pressure (SBP) value was significantly lower in the isoflurane group compared to the sevoflurane group at the time of induction, once the sternum was opened, and once the sternum was closed (P< 0.05). Furthermore, diastolic blood pressure values were significantly lower in the isoflurane group compared to the sevoflurane group at the time of induction, once the sternum was opened (P = 0.004), and once the sternum was closed (P = 0.009). The median heart rate (HR) between the two groups revealed higher values in the isoflurane group, which were statistically significant when compared to the sevoflurane group at time of induction (P < 0.001 ), once the sternum was open (P = 0.001), and once the sternum was closed (P = 0.010). However, the HR values between the two groups did not reveal a statistically significant difference before induction, after Cardiac Care Unit (CCU) admission, and at extubation time (P > 0.05). The median respiratory rate in the isoflurane group was statistically significantly higher when compared to the sevoflurane group at extubation time (P < 0.001).Patients in the isoflurane group had a higher total intubation time compared to patients in the sevoflurane group (P = 0.009). In addition, the median (interquartile range) of the recovery time among patients in the sevoflurane group was significantly shorter than that among patients in the isoflurane group (120 (90-150) min versus 150 (120-180) min, respectively (P-value< 0.001)). Furthermore, regarding the duration of hospital stay, patients who received isoflurane had significantly longer hospital stays (7 [7-8] days) than patients who received sevoflurane (7 [6-7] days), (P = 0.012). Patients in the sevoflurane group were extubated earlier than those in the isoflurane group. Regarding complications that occurred post open heart surgery in patients, there was no significant difference regarding the prevalence of arrhythmias through or post open heart surgery among the two groups. Atrial fibrillation (AF) was seen in the isoflurane group in five (7.9%) patients, and in two patients (3.2%) in sevoflurane group (P = 0.226). Furthermore, no significant differences were noted in the type and incidence of complications regarding bleeding or cerebral vascular attack (CVA). Bleeding post open heart surgery was seen in five (7.9%) patients who were on isoflurane, and in three patients (4.8%) who were receiving sevoflurane (P = 0.372). CVA was reported in two patients (3.2%) in the isoflurane group and in none of the patients in the sevoflurane group (P = 0.254). On the other hand, nausea and vomiting occurred in nine (14.3%) patients who were administered isoflurane, but in only two (3.2%) patients who were administered sevoflurane (P = 0.026). Conclusions: The results reflect that sevoflurane has a better impact on hemodynamic stability than isoflurane. Sevoflurane produces better results in regards to systolic and diastolic pressure, HR, recovery time, ICU stay, analgesia dosage and muscle relaxant dosage. Sevoflurane usage results in less complications than isoflurane, but not significantly.
The clinical effectiveness of the Bispectral Index (BIS) to reduce the incidence of awareness for elective surgical patients undergoing general anesthesia

Background: Unintended intra-operative awareness is a serious complication of general anesthesia. The incidence of such awareness has been reported to be about 0.1-0.6% of patients under general anesthesia. Reminiscence of what occurred during the operation can be felt by patients, which can be stressful and leave lasting mental suffering afterward the operation. Patients that experience unintended intra-operative awareness may have some combination of auditory function, tactual feeling, a sense of weakness, an inability to move, pain, and dread. Bispectral Index (BIS) monitoring has been shown to decrease awareness and boost recovery time from anesthesia. Aims of the study is to evaluate the clinical impact of BIS monitoring to reduce the incidence of awareness and its impact on hemodynamic parameters, drug consumption, the recovery time and the end-tidal concentration of volatile anesthetics in adult patients undergoing various types of surgery under general anesthesia. Methods: The design adopted for this study is a prospective, randomized, double blind trial. The study involved fifty-nine adult patients with American Society of Anesthesiologists (ASA) physical status I-III, aged 18 to 72 years. 41 males and 18 females scheduled for different types of operations under general anesthesia participated in the study. Patients were randomized for inclusion in the BIS-handled anesthesia group (n=30), with the BIS value controlled between 40 and 60, which is considered convenient for surgical anesthesia; or the routine care (RC) group without BIS-control (n=29). A BIS sensor was placed on the forehead of patients. Hemodynamic specifications were recorded before induction of anesthesia and every five minutes during surgery until the removal of the endotracheal tube. The patients were interviewed by a blinded observer at 24-36 hours after operation through the use of a structured questionnaire. Two independent endpoint adjudication committees blinded to group identity assessed the interview results and identified the confirmed awareness cases. Findings: There were no significant differences between the two groups in all the general characteristics of the patients. Regarding anesthetic time, the mean ± SD in the RC group was 76.6 ± 84.3 minutes, and 124.2 ± 124.4 minutes in the BIS group; the difference was not significant. Surgical time was 73.8 ± 85.8 minutes in the RC group and 116.4 ± 106.2 minutes in the BIS group; the difference was not significant. Of the total 59 patients 29 patients were assigned to the routine control group and 30 patients to the BIS group. No case of awareness was reported in the BIS-guided group but 4 reports (13.8%) in the control group (P=0.035), BIS-guided anesthesia decreased awareness by 13.8% (95% CI (1.3%-26.4%). The most common forms of awareness was auditory perceptions, tactile perception and the sense of paralysis. There was a statistically significant difference in the mean dose of inhaled anesthetic agents between the RC group (0.029 ±0.008 %) and the BIS group (0.025 ± 0.009%), P=-0.023, which indicates that BIS monitoring could reduce the needed use of inhalation anesthesia. Regarding the opioid fentanyl there was also a significant difference in the used dose of fentanyl for the BIS group (115.56 ± 94.18 mcg and the RC group (77.76 ± 40.52 mcg), P=0.035.There was found to be a difference in the propofol dosage between the BIS group (474.07 ± 711.3 mg) and the RC group(230 ± 59.938 mg), P=0.235. It is clear that patients in the RC group had a lower dosage of propofol than patients in the BIS group, but the difference was not significant. Low doses of fentanyl and propofol may be one of the causes of awareness in the RC group. We found no significant differences in somatic responses of sweating, tearing, pupil dilation and coughing intra-operatively between BIS-monitored and RC patients. However, a significant reduction in intra-operational jerking was recorded for the favor of BIS group .The percentage of patients who experienced jerking movements intra-operatively was 27.6% in the RC group and 6.9% in the BIS group, P=0.037. There were no statistically significant differences between the two study groups in any of the time measures under study which are: time from cessation of inhalational agents to eye opening; time to respond to commands; time to eye opening (either spontaneously or in response to command, time to first movement response; and time to extubation. The time to phonation for the RC group was 12.82 ± 6.11 minutes and only 10.21 ± 5.127 minutes for the BIS group, P=0.026, this occurs for the favor of BIS group. There is a statistically significant difference between the two groups in the time to discharge from the PACU at 12.38 ± 4.989 minutes for the RC group and 9.23 ± 3.819 minutes for the BIS group, P=0.007. In other words, patients in the BIS-monitored group were discharged earlier from the Post Anesthetic Care Unit (PACU) than the RC patients. Conclusions: BIS-guided anesthesia where the BIS score is kept between 40 and 60, reduced the risk of awareness compared to routine care. The main reason for the occurrence of awareness in the RC group could be due to a light general anesthetic. In addition, BIS monitoring reduces the usage of volatile anesthesia and the time of discharge from the Post Anesthetic Care Unit. Keywords Awareness; General anesthesia; BIS; Monitoring
Comparative Study of Hyperbaric Bupivacaine Plus Ketamine vs. Bupivacaine Plus Fentanyl for Spinal Anesthesia During Cesarean Section. A prospective, Randomized, Double-Blind, Controlled Study

الخلفية: التخدير النخاعي هو الطريقة المفضلة لتخدير الولادة القيصرية، ولكن إدارة الألم بعد الجراحة هي أولوية رئيسية للنساء اللواتي يخضعن للولادة القيصرية. يرتبط عدم كفاية إدارة الألم في فترة ما بعد الجراحة الحادة بالألم المستمر وزيادة استخدام المواد الأفيونية وتأخر الانتعاش الوظيفي وزيادة اكتئاب ما بعد الولادة. في هذه الدراسة، قمنا بتقييم فعالية الكيتامين داخل النخاع مقارنة بالفنتانيل داخل النخاع، على مدة استخدام التسكين لتقليل المواد الأفيونية بعد الجراحة في العملية القيصرية الاختيارية. الطريقة: هذه الطريقة مزدوجهالتعميه، وتم توزيع 105 بشكل عشوائي الى ثلاث مجموعات المجموعة الاولى تلقت 10 ملغم من دواء البوبافكين ممزوج مع دواء الفينتانيل، المجموعة الثانية تلقت 10 ملغم من دواء البوبافكين لوحده وايضاً المجموعة الثالثة تلقت 10ملغم من دواء البوبافكين ممزوج مع 15 ملغم من دواء الكيتامين، تم تسجيل الاثار الجانبية لدراسة: من غثيان، وقئ، انخفاض النبض، هبوط الضغط، وجع بالرأس، حكة، رجة، ورضا المريض، تم قياس بداية والمدة الحسية والحركية ومده التسكين، والعلامات الحيوية قبل، اثناء وبعد عملية الولادة القيصرية التي اجريت تحت التخدير النصفي، تم التسجيل كل 5 دقائق أثناء العملية الجراحية وكل 5 دقائق لمدة 15 دقيقة في وحدة (العناية ما بعد التخدير). النتائج: اظهرت الدراسة انه لا فرق في المعلومات الجغرافية بين المجموعات. كان المريض الذي تلقى الفنتانيل سريعًا في بداية الكتلة الحسية بمتوسط (1.57 دقيقة) عل (p <0.001)، وأسرع في بداية الكتلة الحركية بمتوسط (2.68 دقيقة) على (P <0.001)، وكان لديه إطالة ملحوظة في مدة الكتلة الحسية والحركية، في مجموعة الفنتانيل (P <0.05)، وكان لها استطالة في مدة تسكين الالم (P <0.000)، ولم يكن هناك فرق بين الثلاث مجموعات فيما يتعلق بـ: حدوث تباطؤ في دقات القلب أثناء العملية كان هناك 35/1 (2.9٪) في مجموعة الفنتانيل مقابل 1 \ 35 (2.9٪) في مجموعة الكيتامين، كان معدل حدوث انخفاض ضغط الدم أثناء العملية 25 \ 35 (71.4٪) في مجموعة بوبيفاكين مقابل 29 \ 35 (82.9٪) في مجموعة الفنتانيل و30 \ 35 (85.7٪) في مجموعة الكيتامين، كان حدوث الآثار الجانبية والمضاعفات أثناء العملية القيصرية قليلًا في المجموعات الثلاث، حيث لم يحدث الألم والحكة والقيء في أي من المشاركين ضمن المجموعات الثلاث. بعد العملية الجراحية، كانت نسبة حدوث انخفاض ضغط الدم 1 \ 35 (2.9٪) في ثلاث مجموعات وحالات وشدة الارتعاش بعد العملية الجراحية حيث 1 \ 35 (2.9٪) في مجموعة بوبيفاكين، مقابل3\ 35 (8.6٪) في الفنتانيل، مقابل 2\ 35 (5.7٪) في الكيتامين (p>0.58). حدوث وشدة الغثيان بعد العملية الجراحية حيث 1\ 35 (2.9٪) في مجموعة بوبيفاكايين، (قيمة p> 0.36) ولا أحد في المجموعات الأخرى، حدوث آثار جانبية في فترة ما بعد الجراحة، مثل تباطؤ في دقات القلب، والصداع، والحكة، والرعشة، والغثيان والقيء والضيق التنفسي لم يحدث في أي من المشاركين ضمن المجموعات الثلاث. الخلاصة: نستنتج من الدراسة ان عند اعطاء الفنتانيل مع البوفاكين، يعمل على تسريع في بداية الكتلة الحسية وبداية الكتلة الحركية، وعلى إطالة مدة الكتلة الحسية والحركية، وايضاً في مدة التسكين الالم، مقابل الكيتامين مع البوفاكين او البوفاكين لوحده لدى النساء الخاضعات للعمليات القيصرية تحت تأثير التخدير النصفي النخاعي. الكلمات المفتاحية: الفينتانيل، الكيتامين، البوبفاكين، التخدير النخاعي، العمليه القيصرية.
Comparison of different therapeutic regimens in pain management during dressing among burn patients

Background: Pain is a major problem after burns even when high doses of opioids are administered. The study focused on the effect of using twodifferent therapeutic regimens of analgesia (Morphine with oral Celecoxib, Morphine with Intravenous Paracetamol) to relive procedural pain for burns patient compared with control group (full dose of Morphine). Methods: A randomizedcontrol study at burn unit in Rafedia governmental hospital Northern West-Bank, Palestine was carried out. Patients' medical files were used to obtain demographic, medication and clinical information. VAS (Visual analogue scale) used to assess pain post dressing. Descriptive and statisticsanalysis was conducted using Statistical Package for Social Sciences SPSS 19. Results:Ninety patients hospitalized for burn were recruited for the study from Rafedia hospital. The mean ± SD of the patient’s age was 29.69 ± 14.96 years. More than half of the studied patients were males (53/90; 58.8%). The mean total body surface area of the burn in the studied patients was (19.54% ± 10.85%). The most common burn site among the studied patients was lower limb followed by upper limb (21/90; 23.3%) and (18/90; 20 %) respectively. The majority of the studied patients had second degree burn (55/90; 61.1%). The majority of studied patients were having either a scaled burn (44/90; 48.8%) or a flamed burn (37/90; 41.1%). Finally the majority of studied patients were not having any chronic diseases while (22/90;24.4%) were having chronic diseases. Each patient was evaluated for three consecutive daily dressings using three different treatment regimens as analgesics for the dressing; the regimen number one was (0.1) mg/kg IV Morphine, regimen number two was (0.025) mg/kg IV Morphine with (200) mg orally Celecoxib and regimen number three was (0.025) mg/kg IV Morphine with (1) g IV Paracetamol.Every treatment regimen was followed by pain assessment using VAS. One Way ANOVA analysis indicated that there is a significant difference among the three treatment regimens in VAS score (F=22.36, p<0.001, df=2). Post-hoc analysis using Tukey test indicated that both treatment regimens number one and two were not significantly different from each other. However, both treatment regimens one and two were significantly lower than treatment regimen number three suggesting that treatment regimen number 3 is the least effective in reducing pain during dressing in burn patients. Finally, analysis of VAS score of the three treatment regimens among either gender, or among either degree of burn, or type of burn or presence of chronic disease yielded similar significant results. Conclusion: This is a randomize control study indicates thatMorphine alone or Morphine with Celecoxib demonstrates that using Celecoxib in a multimodal analgesic strategy for procedural pain can achieve favorable efficacy in the manage¬ment of pain. IV Acetaminophen is ineffective in reducing opioids consumption in procedural burn pain. In fact, the VAS was significantly higher in IV Acetaminophen with Morphine-treated patients than in treated patients by Morphine or Morphine with Celecoxib. Comparison of Intra-Peritoneal Instillation of Bupivacaine and Morphine Hydrochloride versus Bupivacaine and Magnesium Sulfate for Post-Operative Pain Relief after Laparoscopic Cholecystectomy A Randomized Double-Blind Comparison Study

المقدمة هناك طريقتين متبعات لإزالة المرارة , حيث يتم إزالة المرارة بشكل جراحي أو بالمنظار , وهذه الأيام يتم تفضيل إجراء العملية بالمنظار , لكون المريض يحتاج لمدة اقل للمتابعة في المستشفى بعد العملية , وتعود الوظائف الجسدية بشكل أسرع بسبب محدودية الإجراء الجراحي المتداخل . وعلى الرغم من ذلك, يبقى المريض يعاني من الألم بعد العملية , بسبب التهيج الحاصل على الحجاب الحاجز بعد إطلاق nociceptive input . يعتبر استعمال مسكن للألم متعدد التركيب للسيطر على الألم بعد عملية المنظار لإزالة المرارة من الأساليب الجديدة هذه الأيام للتقليل من المضاعفات المترافقة مع الجرعات العالية في حالة إستعمال مسكن واحد , ومن الأدوية التي تبين لها دور فعال في الحد من الألم (المغنيسيوم سلفيت ), حيث لاتوجد دراسات للمقارنة بين الحقن البرتوني (الصفاقي الداخلي) لمادة البافكيين مع المورفين هايدروكلورايد، وبين الحقن البرتوني للبافكيين مع المغنيسيوم سلفيت، لتقليل الألم بعد عملية ازالة المرارة بالمنظار الجراحي الأهداف هذه الدراسة هدفها مقارنة تأثير المسكن الذي يتم حقنه في الجدار البرتوني (الصفاقي الداخلي) لمادة البافكيين مع المورفين هايدروكلورايد، وبين الحقن البرتوني للبافكيين مع المغنيسيوم سلفيت، لتقليل الألم بعد عملية ازالة المرارة بالمنظار الجراحي, لمن يتم إخضاعهم لتخدير كامل , للوصول بالمريض لأقل مستوى للألم , وللتقليل من الحاجة لاستعمال المسكنات ما بعد العملية خلال الأربعة وعشرون ساعة اللاحقة للعملية. تصميم الدراسة بعد خضوع البحث لموافقة لجنة أخلاقيات البحث العلمي في جامعة النجاح الوطنية وكتابة نموذج موافقة لإخضاع المريض للبحث بعد عملية إزالة المرارة بالمنظار , خضع للتجربة 100 حالة تتراوح أعمارهم بين عمر 18 و 60 عاما , وبدرجة أولى وثانية حسب تصنيف جمعية أطباء التخدير الأمريكية . وبشكل عشوائي تم تقسيم المرضى لمجموعتين , تم إعطاء المجموعة الأول مادة البافكيين 30 مل 0.25 % , مع المورفين هايدروكلورايد 3 ملغم , بالمقابل تم إعطاء المجموعة الثانية والتي احتوت 50 حالة أيضا , مادة البافكيين 30 مل 0.25 % مع المغنيسيوم سلفيت( 50 ملغم / كيلو),اعطاء الدواء تم بعد غسل وسحب السوائل من الجدار البرتوني (الصفاقي الداخلي) ,تم تحضير الدواء من طبيب غير مشارك بالدراسة , وجميع المرضى تم تخديرهم للعملية بنفس البروتوكول , ومن ثم مراقبة تخطيط القلب , النبض , ضغط الدم , ونسبة إشباع الدم بالأكسجين لجميع المرضى , وتم مراقبة المشاركين في البحث لفترة 24 ساعة بعد الانتهاء من العملية لضبط مستوى الألم لديهم . باستعمال مقياس الألم , حيث يتضمن 10 مستويات , تم اعتبار المستوى 4 من مقياس الألم , الحد الفاصل للتدخل وإعطاء المريض جرعة المسكن التي تم اعتمادها من مادة المخدر البيثادين 20 ملغم، عن طريق الوريد, في غرفة متابعة المريض والمراقبة في قسم العمليات وفي حال حاجة المريض لجرعة اضافية للمسكن يتم اعطائه 50 ملغم عن طريق العضل خلال تواجد المريض في قسم الجراحة في حال وجود الألم . النتائج كانت خصائص المرضى من العمر والجنس ومؤشر كتلة الجسم قابلة للمقارنة في المجموعتين. لم يكن هناك فرق كبير بين المجموعتين فيما يتعلق مدة الجراحة. المعلمات الديموغرافية (العمر، الجنس وBMI) ليس لها أي تأثير على متوسط من VAS (ف قيمة> 0.05). هناك فروق ذات دلالة إحصائية بين مجموعات من مو والمغنيسيوم في الدرجة الكلية VAS (قيمة P <0.05). في المجموعة مو، يعني من إجمالي VAS (2:09) كان أقل بكثير من متوسط مجموع المغنيسيوم VAS في مجموعة (2.71)؛ ما يعني مو في المجموعة المريض كان أقل بكثير من شدة الألم في المجموعة المغنيسيوم المريض (ع = 0.006). هناك فرق كبير بين عدد (في المائة) من يشكو المريض من الآلام المتوسطة والشديدة بعد العملية الجراحية في مجموعة مو 15/50 (30٪) بالمقارنة مع مجموعة المغنيسيوم 25/50 (50٪) (ع = 0.0423). عندما تقدير حجم تأثير العلاج من المورفين بالإضافة إلى هيدروكلوريد بوبيفاكايين، وجدت أن الحد من المخاطر النسبية من الآلام المتوسطة والشديدة بعد العمل الجراحي هو 00:40. هو أيضا هناك فرق كبير بين عدد (في المائة) من المريض يشكو من الخمول في المجموعة مو 7/50 (14٪) بالمقارنة مع مجموعة المغنيسيوم 18/50 (36٪) (ع = 0.0115). لا توجد فروق ذات دلالة إحصائية بين مجموعتي الدراسة فيما يتعلق الغثيان والتقيؤ والدوار واحتباس البول. المرضى في المجموعة مو تستهلك أقل الإنقاذ جرعة مسكن M (± SD) (64.29 ملغم + 22.04) بالمقارنة مع المريض في مجموعة المغنيسيوم M (± SD) (74.40 ملغم + 25.67) دون علاقة ذات دلالة إحصائية بين الجرعات عناء (قيمة ص = 0.163) . تم فحص ضغط الدم ومعدل ضربات القلب وتشبع الأكسجين كمعلمات الدورة الدموية. أظهرت النتيجة عدم وجود علاقة ذات دلالة إحصائية بين هذه المعلمات و VAS (قيمة p> 0.05). الاستنتاج تقطير داخل الصفاق من مزيج من بوبيفاكايين مع هيدروكلوريد المورفين متفوقة على بوبيفاكايين بالإضافة إلى المغنيسيوم سلفات للحد من شدة وحدوث آلام ما بعد الجراحة في المستشفى خضع لجراحة استئصال المرارة بالمنظار دون زيادة كبيرة من الآثار الجانبية. يوفر هذا التأثير المحيطي للأفيونيات نهجًا جديدًا لتخفيف الآلام يمكن أن يكون له فوائد سريرية كبيرة. توصية وبناء على نتائج هذه الدراسة، فمن المستحسن للنظر في تقطير داخل الصفاق من هيدروكلوريد المورفين مع بوبيفاكايين كتطبيق افتراضي لجراحة استئصال المرارة بالمنظار للحد من الألم بعد العملية الجراحية لإزالة المرارة بالمنظار
A Comparison of Pre-Emptive Effect of Oral Pregabalin and Gabapentin on Acute Postoperative Pain in Patients Undergoing Lumber Spine Surgeries

المقدمة: تعتبر آلام أسفل الظهر سببًا رئيسيًا لعمليات العمود الفقري السفلي ، لذلك يخضع المرضى لهذا الإجراء لتخفيف الآلام المتوقعة. ومع ذلك ، فإن تلف الأنسجة أثناء الجراحة هو السبب الرئيسي لتطور آلام ما بعد الجراحة المزمنة ، وعندما يصبح الألم مزمنًا ، تحدث تغيرات أساسية في الأنماط الظاهرية للخلايا العصبية ودوائر الدماغ. يمكن أن تغير هذه التغييرات المراكز الحسية والعاطفية والتحفيزية في الدماغ وتتداخل اليومية بين الغثيان والقيء بعد الجراحة ، والتأخر في الذهاب إلى غرفة العمليات والعوامل الاجتماعية. لذلك ، يجب أن يكون تخفيف الآلام المناسب بعد الجراحة ونظام المسكنات متعدد الوسائط جزءًا لا يتجزأ من إدارة التخدير. الأدوية المتوفرة حاليًا لعلاج الألم الحاد غير فعالة في الغالب في الوقاية منه وغالبًا ما يتم الإفراط في استخدام المواد الأفيونية في فترة ما بعد العملية الجراحية، مما يتسبب في مضاعفات لجابابنتين عن طريق الفم مقابل بريجابالين الفموي في المرضى الذين يخضعون لعمليات جراحية في العمود الفقري السفلي تحت التخدير العام. تم اختيار تصميم الدراسة ليكون الأنسب لأهداف الدراسة بالإضافة إلى التدخل المقدم المتعلق بنتائج التدخل المقاسة. من المقرر إجراء الدراسة في وحدة العمليات ، وحدة التعافي ، وأجنحة ما بعد الجراحة في مستشفى رفيديا الحكومي الجراحي في نابلس ، فلسطين. يشمل مجتمع الدراسة المرضى الذكور والإناث الذين يخضعون لعمليات جراحية اختيارية في العمود الفقري في قسم طب الأعصاب. الحالة الجسدية للمرضى الأول والثاني حسب تصنيف الجمعية الأمريكية لأطباء التخدير ، تتراوح أعمارهم من 18 إلى 60 عامًا ، ويخضعون لعمليات جراحية اختيارية في العمود الفقري في إطار قسم طب الأعصاب في مستشفى رفيديا الحكومي الجراحي وتلبية معايير التضمين. تم تقسيم المرضى (60 مريضًا) الذين يستوفون معايير الاشتمال إلى 3 مجموعات بطريقة عشوائية بسيطة: تلقت المجموعة بريجابالين 150 مجم ، تلقت المجموعة جابابنتين 300 مجم وتلقي المجموعة العلاج الوهمي مع رشفات من الماء قبل ساعة واحدة من الجراحة من قبل احد أعضاء فريق الرعاية. لم يكن المرضى ، وجميع الموظفين المشمولين في رعاية المرضى ، والشخص الذي يجمع البيانات ، والمحكمين على النتائج على دراية بتخصيص مجموعات العلاج. تم تقسيم المرضى المسجلين في الدراسة بشكل عشوائي إلى واحدة مجموعتي البريجابالين و الجابابنتين مقارنة بمجموعة الدواء الوهمي وتخفيف الآلام بشكل ملحوظ لفترة أطول من البريجابالين عند مقارنته مع الجابابنتين وحده كانت أعلى كمية من الباراسيتامول المسكن المعطى في مجموعة الدواء الوهمي مقارنة بمجموعتي بريجابالين وجابابنتين. و كانت أقل كمية من الباراسيتامول المعطى في مجموعة بريجابالين. كانت نسبة الغثيان والقيء بين المشاركين في مجموعة الدواء الوهمي أعلى مقارنة بالمشاركين في مجموعة جابابنتين (80٪ مقابل 40٪) ومجموعة بريجابالين (80٪ مقابل 35٪). كانت درجات التخدير أعلى في أول 6 ساعات بعد الجراحة في مجموعتي البريجابالين و الجابابنتين مقارنة بمجموعة الدواء الوهمي. الخلاصة: ثبت أن بريجابالين الوقائي (150 مجم) أكثر فعالية من جابابنتين (300 مجم) ، في إطالة مسكنات الألم بعد العملية الجراحية مع انخفاض متطلبات مسكنات الإنقاذ. على الرغم من ملاحظة التخدير ما Dexamethasone and Metoclopramide, and their Combination for the Prevention of Postoperative Nausea and Vomiting in Female Patients Undergoing Laparoscopic Surgery

Introduction: No single antiemetic drug has proven to be a universal solution to postoperative nausea and vomiting. There is evidence however, that in patients with a high risk of developing PONV, combination antiemetic therapy is considered to be a viable assistive practise. The aim of this study is to evaluate the effect of prophylactic dexamethasone and metoclopramide as monotherapies and in combination for female patients with moderate to high risk for PONV related to laparoscopic surgery under general anesthesia. Methods:A multicenter, prospective, randomized, double-blind, placebo controlled trial was used in this study.One hundred twenty female patientswere allocated randomly to one of the four groups, with thirty patients in each: (D) group received 8 mg dexamethasone one minute immediately before induction of anesthesia and 10 ml saline before end of anesthesia.,(M) group received 10 ml saline one minute immediately before induction of anesthesia and metoclopramide 20 mg before end of anesthesia, (C) group received dexamethasone (8 mg) one minute immediately before induction of anesthesia and metoclopramide (20 mg) before end of anesthesia, and (P) group received 10 ml saline one minute immediately before induction of anesthesia and 10 ml normal saline before end of anesthesia. Postoperative symptoms were assessed postoperatively. Results: Significant differences in the incidence of vomitingwithin 24 hrs postoperatively between placebo group17 (56.7%) and combination group 6 (20%), P = 0.0037, and dexamethasone group 6 (20%), P = 0.0037 were demonstrated. The results show that the incidence of vomiting was reduced significantly by use of dexamethasone as amonotherapy or in combination with metoclopramide. Significance differences were also noted in the incidence of nausea 24 hrspostoperatively between the treatment groups (D, M, and C) when compared with placebo group p< 0.05. There were no statistical significance differences between the three treatment groups, p>0.05. Results can be interpreted as evidence that using metoclopramide and dexamethasone alone or in combination in this sample decreased the incidence of nausea during the 24 hours after recovery as compared with a non-active placebo. Significant differences were exhibited in the intensity of nausea 24 hours postoperative between the combination group 1.05 (± 0.90) compared with dexamethasone 1.78 (± 1.14), P = 0.008, Metoclopramide 2.43 (± 1.03), P = 0.000 and the placebo group 3.07 (± 1.34), P=0.000. These results demonstrate that using a combination of dexamethasone and metoclopramide canreducs significantly the intensity of nausea in the critical 24 hour postoperative period. There were noted significant differences in need to administer antiemeticin the 24 hour postoperative period.The total number of patients who received the combination therapy 6(20%)and dexamethasone 7(23.3%) required antiemeticat a lower rate than those who received placebo 19(63.3%), p < 0.05. There was no significant difference evidenced between placebo and metoclopramide 13 (43%) p= 0.1237. The results show that use of either dexamethasone or combination reduced significantly the need for additional analgesic medication 24 hours post-operation. Reported incidence of pain24 postoperativein combination therapy was 22 (77.3%) and in the dexamethasone group 21 (70%) compared to the placebo group 29 (96.7%), p< 0.05 and the metoclopramide group 28 (93.3%), p< 0.05. The results indicate that the incidence of pain was reduced significantly in the combination and dexamethasone groups. Conclusion:The combination of dexamethasone plus metoclopramide appears to be a more effective prophylactic in reducing the intensity of nausea than dexamethasone, metoclopramide alone or a placebo. Dexamethasone and the combination of dexamethasone plus metoclopramide were more effective in preventing PONV and decreasing the severity of postoperative pain than metoclopramide alone and placebo. Patients who are at high risk for PONV may demonstrate positive results when using a combination therapy. In addition no adverse events related to use were found. Dose Response Study of Intrathecal Fentanyl Added to Bupivacaine in Patients Undergoing Elective Caesarean Section in Spinal Anesthesia

Background Applying spinal anesthesia for caesarean sections associated with a certain side effect such as hypotension caused by the dose of hyperbaric bupivacaine, additives of potent opioid such as fentanyl may reduce the dose of toxic local anesthesia which will be more hemodynamic stability and also enhance of adequate analgesia. Aims This study conducted in order to evaluate the side effects of four spinal solutions in purpose of finding best possible combinations of fentanyl and bupivacaine, comparing the efficiency and safety of this combinations by using different doses of fentanyl in spinal anesthesia in cesarean section, and assessing the side effects, duration of analgesia, hemodynamic parameters and neonatal outcomes by using an Apgar score. Methods One hundred sixty Patients randomized into four groups 40 of each : Group-I (F10): received 1.5ml (7.5mg) of 0.5% Bupivacaine heavy & 10μg Fentanyl .Group-II (F15): received 1.5ml (7.5mg) of 0.5% Bupivacaine heavy & 15μg Fentanyl .Group-III (F25): received 1.5ml (7.5mg) of 0.5% Bupivacaine heavy & 25μg Fentanyl and (control group ). Group-IV (B10): received 2 ml (10mg) of 0.5% Bupivacaine heavy and saline containing no fentanyl. Side effects such as: nausea, vomiting, bradycardia, hypotension episodes, headache, pruritis, shivering, drowsiness, restlessness, dizziness, sedation, patients’ satisfaction, analgesic requirements and duration of effective analgesia were evaluated. Moreover, onset and duration of sensory and motor block were measured. Results Similar distribution of age, height, weight and duration of surgery as evidenced by statistical analysis, The results show that there was significant differences at the level (p ≤ 0.05) in comparison between B10 only and Fentanyl 10mcg, Fentanyl 15mcg and Fentanyl 25mcg related to the onset of sensory blockade to T10 and T6 and indicate fentanyl 25 mcg more fast onset of sensory block. Onset of motor block was earlier in bupivacaine 10 mg group in comparison with other three groups, which is statistically significant difference. According to bradycardia, there were no significant differences between groups. Hypotension episodes in bupivacaine 10mg more common mean (3.12), which is statistically significant difference when compared to other three groups. Incidence of headache was decreased in fentanyl groups in comparison with B10 group but there were no significant differences between the groups. There were significant differences regarding the incidence of pruritis in fentanyl 25 mcg (9/40) and fentanyl 15 mcg (6/40) groups when compared to B10. Nausea and vomiting were observed in B10 group while decreased significantly in other three groups. According to Restlessness, sedation and respiratory depression, there were no significant differences between all groups. There were no significant differences in Apgar scores at 1 and 5 min. The duration of sensory block was prolonged significantly in fentanyl groups. Duration of motor blocked was decreased significantly with decrease Bupivacaine dose and early motor recovery was observed significantly in fentanyl groups compared to B10 group. Duration of effective analgesia increased as increased fentanyl dose which was statistically significant difference in all fentanyl groups compared to B10 group. Regarding to postoperative analgesic needed, significant differences were found in all fentanyl groups in comparison to control group, fentanyl 25 mcg more significant and less rescue analgesic requirements in post-operative period by mean (1.85 ) followed by fentanyl 15 ( 2.3) mcg then B10 (2.37) and fourthly fentanyl 10 mcg (2.42 ) . Conclusion Addition of Fentanyl was effective with minimal side effects; also it improves the quality of anesthesia, prolongs the duration of sensory block, faster onset of sensory block and significantly reduces post-operative rescue analgesic needed, also better hemodynamic stability. Where 0.5% bupivacaine 10 mg alone faster of onset of motor blocked and prolonged of duration of motor blocked. Finally we conclude and recommend of best combination in spinal anesthesia for elective cesarean section is fentanyl 25 mcg with 0.5% bupivacaine 7.5 mg because it was superior in duration of effective analgesia and less post-operative rescue analgesic needed also more patients’ satisfaction, however may have less side effect in fentanyl 10 mcg and 15 mcg groups. In conclusion fentanyl 25 mcg with 0.5% bupivacaine 7.5 mg is more favorable.

المزيج بين الليدوكائيين القلوي والديكساميثازون داخل كفة الأنبوب الرغامي لديه الافضليه والفائدة في تقليل حدوث السعال في مرحلة الافاقه. الليدوكايين القلوي والخليط بين الليدوكائيين القلوي والديكساميثازون متساويين في تقليل حدوث السعال وتقرح الحلق وبحة الصوت في جميع المراحل ابتداء من ساعتين بعد وحدة ما بعد التخدير وحتى 24 ساعة . الديكساميثازون ليس ذو فائدة في تقليل حدوث تقرح الحلق عند الساعتين و8 ساعات بعد وحدة ما بعد التخدير وكذلك ليس ذو فائدة في تقليل بحة الصوت عند الثمان ساعات. ولكنه يمتلك نفس التأثير لتقليل حدوث السعال تماما مثل الليدوكائيين القلوي والخليط بين الليدوكائيين القلوي والديكساميثازون. بالنسبة لشدة حدوث الأعراض /السعال/ وتقرح الحلق/ وبحة الصوت. فان المجموعات الثلاث /الليدوكائيين القلوي/ والديكساميثازون/ والخليط بين الليدوكائيين القلوي والديكساميثازون أفضل من الهواء لتقليل شدة الأعراض عند وقت الساعتين بعد وحدة ما بعد التخدير. بالنسبة لشدة حدوث بحة الصوت فان الليدوكائيين القلوي هو الأفضل لتقليله. الليدوكائيين القلوي والخليط بين الليدوكائيين القلوي والديكساميثزون يعمل على تحسين انسجام القصبة الهوائية للأنبوب الرغامي ويجعل عملية نزع الأنبوب أكثر سهوله. وبشكل عام نستطيع القول أن الليدو كائيين القلوي والخليط بين الليدوكائيين القلوي والديكساميثازون داخل كفة الأنبوب الرغامي أفضل من الديكساميثازون والهواء. ولا يوجد فرق بين الليدوكائيين القلوي والخليط بين الليدوكائيين القلوي والديكساميثازون. تطبيقات تمريض التخدير: المزيج بين الليدوكايين القلوي والديكساميثازون أو الليدوكايين القلوي لوحده داخل كفة الأنبوب الرغامي يؤدي إلى تأثيرات جيده لمنع حدوث اعتلالات القصبة الهوائية )السعال وتقرح الحلق وبحة الصوت( بعد نزع الأنبوب. بالإضافة إلى جعل عملية نزع الأنبوب أكثر سهوله واقل خطورة. الكلمات المفتاحية: التدخين. الليدوكائين. الديكساميثازون. السعال. بحة الصوت. تقرح الحلق.
The Effect of Femoral Nerve Block on Opioid Consumption among Patient’s who Underwent Total Knee Replacement under Spinal Anesthesia

Background Total knee replacement (TKR) is a common surgical procedure which has become the treatment of choice for people with intractable joint pain and disability. It’s a clinically efficacious and cost-effective intervention, with high rates of success in terms of alleviating pain and improving function in patients with advanced arthritis of the knee. The post-operative complications may delay the mobility and discharge. There are many complications such as traumatic to nerves or blood vessels in the leg, dislocation of the prosthesis, allergic to the cement material and pain post operation. Pain management is an important part of recovery. The multi module post-operative pain management continuous epidural, lumbar plexus, and femoral or sciatic nerve blocks improve postoperative pain management and reduce opioid consumption. Femoral nerve block (FNB) is one of the methods used to decreased opioid consumption. A femoral nerve block can be used to provide anesthesia for the anterior thigh, knee, and a small part of the medial foot. It is typically used in conjunction with other lower extremity blocks. It may also be used for postoperative pain relief following knee surgery. Aims To assess the efficacy of FNB in reducing pain among spinal anesthesia total knee replacement, the efficacy of FNB in reducing post-operative nausea and vomiting (PONV) among spinal anesthesia total knee replacement, and to assess the efficacy of femoral nerve block in reducing opioid consumption post-operative spinal anesthesia total knee replacement. Methods A randomized controlled trial (RCT) conducted at Rafidia hospital. The patients participating in the trial were randomly assigned to either group received the femoral nerve block under investigation (intervention group) or to a group did not receive treatment (control group). The sample size based on α 0.05 and affect referred size and power 0.80. The estimated sample size is (100 participants). Results Of the 100 subjects who underwent unilateral total knee arthroplasty (TKA) included in this study, 44% (n = 44) of subjects had received a femoral nerve block (FNB) prior to total knee arthroplasty (TKA), while 56% (n = 56) did not receive a femoral nerve block prior to surgery. The two groups, femoral nerve block vs. non-femoral nerve block, showed differences in the percentage of reported postoperative surgery outcome parameters variables including nausea, vomiting, and drowsiness. Non-femoral nerve block group reported higher percentage of post-operative surgery nausea than femoral nerve block group at all post-operative surgery measured time intervals except at the 4th post-operative hour. The percentage of vomiting by non-femoral nerve block group at the 4th and 24th post-operative hours was lower (26.8% & 3.6%) than the reported percentage of vomiting by femoral nerve block group (27.3% & 4.5%). The two groups, femoral nerve block vs. non-femoral nerve block, showed no statistically differences in the mean of systolic blood pressure or diastolic blood pressure at pre and all post-operative surgery measured time intervals except at the 12th post-operative hour for systolic and diastolic blood pressure (p value < 0.05) and pre-operative heart rate (p value < 0.05). The mean of systolic blood pressure and diastolic blood pressure at 12th hour post-operative were 120.5 ± 12.9 mmHg and 70.0 ± 7.1 mmHg for non-femoral nerve block group and were 128.1±9.3 mmHg and 72.7±4.0 mmHg for femoral nerve block group The two groups, femoral nerve block vs. non-femoral nerve block, showed no statistically differences in the mean of heart rate at all post-operative surgery measured time intervals except at the pre-operative heart rate (p value < 0.05). The two groups, femoral nerve block vs. non-femoral nerve block, showed no statistically differences in the mean of O2 saturation at pre and all post-operative surgery measured time intervals (p value > 0.05). The overall pain relief among patients’ femoral nerve block group following TKR surgery was higher comparing with pain relief among non-femoral nerve block group after the surgery. There is a significant difference in post-operative surgery analgesia consumption between femoral nerve block and non-femoral nerve block. A Mann-Whitney test indicated that post-operative surgery analgesia consumption was greater for non-femoral nerve block group than femoral nerve block group (U = 66, Z score= -7.85, p = .001). The overall satisfaction of patients’ femoral nerve block group following TKR surgery was higher comparing with overall satisfaction of non-femoral nerve block group. Conclusion This study compared post-operative nausea vomiting and drowsiness pain scores, opioid consumption, and patient satisfaction between subjects who received femoral nerve block with non-femoral nerve block in total knee replacement. This study found the overall satisfaction with the function of their operation knee among patients’ femoral nerve block group following total knee replacement surgery was higher comparing with overall satisfaction with the function of their operation knee among non-femoral nerve block group after the surgery. And pain relief among patients’ femoral nerve block group following total knee replacement surgery was higher comparing with pain relief among non-femoral nerve block group after the surgery some trends in the data, do suggest efficacy in the use of a femoral nerve block for total knee replacement
THE EFFECT OF INTRAOPERATIVE CONTINUOUS INFUSION OF ESMOLOL ON PAIN REDUCTION, VOMITING AND HEMODYNAMICS STABILITY POSTOPERATIVE ON LAPAROSCOPIC CHOLECYSTECTOMY PATIENTS

Background: Alleviation of postoperative pain often requires administration of potent analgesics like opioids. Reduction of opioid analgesic doses, incidence, frequency, and intensity of postoperative pain can improve patient outcomes. Objective: This study was conducted to assess the effects of continuous intraoperative infusion of esmolol on reducing postoperative pain among patients who underwent laparoscopic cholecystectomy under general anesthesia. The study also assessed the effects of continuous intraoperative infusion of esmolol on reducing nausea and vomiting among the included patients. also the associations between demographic characteristics of the patients and the effects of continuous intraoperative infusion of esmolol on reducing postoperative pain, duration of the laparoscopic cholecystectomy operation, and degree of postoperative pain were also investigated. Methods: This study was conducted using a double-blind randomized controlled clinical trial design. Patients in both control and intervention groups were adults (>18 years old) who were recruited from Rafedia hospital. Patients in the intervention group started on continuous intraoperative infusion of 5-10mcg/kg/min esmolol until the completion of surgery. In the control group, patients received n/s0.9% at same rate. Postoperative pain was measured by visual analogue scale (VAS). Demographic and hemodynamic variables of the patients were collected on an assessment sheet that was developed for this study. Results: A total of 65 patients were randomly allocated into control (n = 36) and intervention (n = 29) groups. There were no statistical differences in the demographic data and preoperative hemodynamic variables of the patients in both groups before the intervention was administered. In this study, esmolol continuous intraoperative infusion was shown to maintain PACU hemodynamic parameters and significantly reduced postoperative pain (up to 1 hour postoperatively) among patients who underwent laparoscopic cholecystectomy. The average time to require the first dose of rescue analgesia was longer in the esmolol group compared to the control group. However, this difference was not statistically significant. Conclusion: Postoperative pain continues to present a heavy burden on patients who undergo surgical interventions, notably, laparoscopic cholecystectomy. In conclusion, continuous intraoperative infusion of esmolol during maintenance anesthesia of patients undergoing laparoscopic cholecystectomy was shown to significantly reduce postoperative pain without destabilizing the hemodynamic parameters. Furthermore, rescue analgesia was less frequently needed in the esmolol group, nausea and vomiting were not reduced by esmolol and are still a major concern. Results of this study might be used to improve future perioperative care of patients scheduled for laparoscopic cholecystectomies.
Effect of Prophylactic Ondansetron on the Incidence of Spinal-anesthesia-induced Shivering and Hypotension in Elective Cesarean Sections: A Prospective, Randomized, Placebo-Controlled, Double-Blind Study

Background: Spinal anesthesia is the preferred method of anesthesia for cesarean section, but it is associated with dangerous adverse effects to the mother and fetus, this includes: hypotension and shivering. Studies suggest serotonin may have role in hypotension, bradycardia and shivering occurrence. In this study, we evaluated the efficacy of ondansetron, a serotonin receptor antagonist, on the incidence of spinal-anesthesia-induced shivering, hypotension, nausea, vomiting and other complications in elective cesarean sections. Methods: this was double blinded RCT, were 80 full-term parturient were randomly allocated into two groups, immediately before induction time, the treatment group received an IV bolus of 4 mg ondansetron and the control group received IV 10 ml of 0.9% saline. Study observations and hemodynamic parameters were recorded pre-, intra-, and postoperatively: every 3 min intraoperatively and every 5 min for 15 min in the post-anesthesia care unit. Results: there was significant difference between both study groups in regard to: incidences of intraoperative hypotension and dizziness there were 25/40 cases (62.5%) in control group vs 9/40 cases (22.5%) in ondansetron group (P < 0.001), incidences and intensity of intraoperative shivering there were 13/40 cases (32.5 %) in control group vs 5/40 cases (12.5 %) in ondansetron group (P = 0.032). Intraoperative nausea intensity was lower in ondansetron group (P = 0.049). Postoperatively, incidences of dizziness where 15/40 cases (37.5 %) in control group vs 2/40 cases (5 %) in ondansetron group (P =0.001), incidences and intensity of postoperative shivering where 15/40 cases (37.5 %) in control group vs only 5/40 cases (12.5 %) in ondansetron group (P = 0.010). Incidences and intensity of postoperative nausea where 16/40 cases (40 %) in control group vs only 7/40 cases (17.5 %) in ondansetron group (P = 0.026), postoperative vomiting, incidences where 9/40 cases (25.5 %) in control group vs only 1/40 cases (2.5 %) in ondansetron group (P = 0.014). Conclusion: prophylactic 4 mg IV ondansetron significantly attenuates the incidence of spinal anesthesia-induced shivering and hypotension, dizziness, nausea and vomiting occurrence. Effect of Spinal and General Anesthesia over Apgar score and blood PH of umbilical cord in Neonates Born after Elective Cesarean Section in Jenin governmental hospital

Introduction: During pregnancy many physiological changes happens to prepare mother for labour and delivery, and developing of the fetous include renal vasodilatation, cardiac changes, renal plasma flow and glomerular filtration rate both increase. There is a rising in the number of woman underwent caesarean section, and this was noted in all over countries in the world. Different anesthetic method can be used to perform the C/S, as general or regional. The choice of anesthesia for cesarean section depends upon indication for operation, its urgency, or patients, obstetrician and anesthesiologist preferences. Aim: To compare the effect of general anesthesia and spinal anesthesia on Apgar score and umbilical cord PH to newborns baby to patients undergoing elective C/S. Material and methods: interventional study was used, to investigate which anesthesia type (spinal or general) have minimal or no effect over umbilical cord PH and Apgar score baby born after elective caesarian section. It was done in Jenin Governmental Hospital, in operation ward and recovery room. Randomization technique was used in this study. The sample size was (103) dividing in two groups 53 participants in G/A and 50 participants in spinal anesthesia. The following information was obtained from medical records for analysis: Demographic data , type of anesthesia ,obstetric factors gender, past medical history to mothers, past surgical history, newborn status, Apgar score to baby, Blood analysis PH: which taken from arterial cord blood. The neonatologist do assessment to baby, and researcher sent PH sample to analysis. Result: Regarding result in the study showed that the average of Apgar score at one minute in G /A group was 7.4 ±.94, in spinal anesthesia was 8.3±1.01 (p< 0.001). The average of Apgar at minute 5 in General anesthesia group was 9.9±0.26, and in Spinal anesthesia was 9.9±0.44 (p=0.95). Umbilical artery blood PH of umbilical cord blood sample of newborns in general anesthesia group was acidosis or alkalosis 11.5% and 3.8% respectively ,while it was in spinal anesthesia group acidosis and alkalosis (2.1% and 0.0%) respectively. Conclusion and recommendation: Spinal anesthesia is safer than general anesthesia, for both mother and fetus; it's associated with better neonatal out come as compared to general anesthesia. Apgar score and PH is a reliable predicator for neonate outcome. Umbilical cord arterial blood gases should be taken immediately after delivery from the umbilical cord to newborn has low Apgar score. One minute Apgar score in spinal anesthesia group was better and fetal acidosis was lower than general anesthesia group.
Effect of Surgical Site Infection Bundles in Cardiac Surgery: Incidence, Microbiology, and Risk Factors

مقدمة: التهابات الموقع الجراحية، التي حددتها مراكز السيطرة على الأمراض والوقاية منها على أنها "عدوى تحدث بعد الجراحة في منطقة الجسم التي أجريت فيها الجراحة، هي ثاني أكثر أنواع العدوى المرتبطة بالرعاية الصحية شيوعًا. الهدف: الهدف العام من هذه الدراسة هو معرفة مدى الإصابة وعوامل الخطر المرتبطة بعدوى الموقع الجراحي بعد تطبيق معايير الوقاية من الالتهابات بعد العمليات الجراحية بعد الجراحة القلبية التي تشمل الجرح القصي والساق. جمع البيانات: الأسلوب الوصفي الكمي بما في ذلك التحليل بأثر رجعي والمتابعة لمرضى جراحة القلب والاستخدام التجريبي المحتمل لنهج حزم المواقع الجراحية مقارنة بالرعاية الروتينية في التقييم بأثر رجعي في المستشفيات الحكومية والخاصة في الضفة الغربية. شمل ما مجموعه 222 مريض جراحة القلب في هذه الدراسة، 110 منها للمراقبة و112 للتجربة. النتيجة: من بين المرضى الذين خضعوا لعملية جراحية في القلب 70٪ خضعوا لعملية تحويل مسار الشريان التاجي، ومعظمهم (90.5٪) تزيد أعمارهم عن 45 عامًا. علاوة على ذلك، كان الذكور سائدين (70.3٪)، و45٪ منهم إما يعانون من زيادة الوزن أو السمنة. كان ارتفاع ضغط الدم هو الأكثر انتشاراً (72.5٪) من الأمراض المزمنة. كان معدل حدوث التهاب الموقع الجراحي في دراستنا 7.2٪. ارتبط التقيد ببروتوكول الوقاية الجراحية (الحزمة) بانخفاض في معدلات الالتهابات للموقع الجراحي، وكان معدل حدوثها أعلى بين المجموعة التقليدية مقارنة بمجموعة التجريبية لحزمة الوقاية (10.9٪ مقابل 3.6٪). كانت اكثر انواع البكتيريا المسببة للعدوى هي المكورات العنقودية الذهبية المقاومة للميثيسيلين والمكورات العنقودية الذهبية والمكورة العقدية المقيحة. خاتمة: ارتبط التقيد ببروتوكول الوقاية الجراحية (الحزمة) بانخفاض معدلات مباحث أمن الدولة. كان معدل حدوث SSI أعلى بين المجموعة التقليدية مقارنة بمجموعة حزمة SSI (10.9٪ مقابل 3.6٪). كانت اكثر انواع البكتيريا المسببة للعدوى هي المكورات العنقودية الذهبية المقاومة للميثيسيلين والمكورات العنقودية الذهبية والمكورة العقدية المقيحة. كان متوسط مستوى الجلوكوز المرتفع، وارتفاع مؤشر كتلة الجسم، والإصابة بأمراض الجهاز التنفسي المزمنة عوامل مستقلة مرتبطة بـحدوث الالتهابات في الموقع الجراحي في المرضى الذين خضعوا لإجراءات جراحة القلب. لم يكن للعمر والجنس والسكري والفشل الكلوي المزمن أي دلالة إحصائية، ولكن جميعها كانت معرضة لخطر تطوير مباحث أمن الدولة. الكلمات المفتاحية: حزمة الرعاية، جراحة القلب، عدوى الموقع الجراحي، مسببات الأمراض، الجروح القصية والساق.
Effects of Dexamethasone, Lidocaine, Dexamethasone and Lidocaine Together on Patient Undergoing Laparoscopic Cholecystectomy to Prevent Sore Throat, Cough and Hoarseness

كان الهدف من هذه الدراسة هو البحث في تأثير إستخدام دواء ديكساميثازون، ليدوكايين، وكلاهما معا، في منع التهاب الحلق والسعال وبحة الصوت لدى المريض الذي يخضع لعملية استئصال المرارة بالمنظار. وقد أجريت الدراسة في مستشفى رفيديا - نابلس ومستشفى جنين الحكومي، على عينة من المرضى الذين خضعوا لعملية استئصال المرارة بالمنظار، وتضمنت العينة (60) مريضاً تم اختيارهم عشوائياً من مجتمع الدراسة، واعتمدت الدراسة على تصميم الدراسة المستقبلية، وكانت الأداة هي الملاحظة في نفس يوم العملية من 3-24 ساعة. أظهرت النتائج أن المجموعة التي تناولت مزيج الديكساميثازون والليدوكايين أظهرت انخفاضًا في نسبة حدوث التهاب الحلق والسعال وبحة في الصوت لدى المرضى الذين خضعوا لعملية استئصال المرارة بالمنظار. أظهرت هذه المجموعة أفضل التأثيرات تليها المجموعة التي تناولت الديكساميثازون بمفرده والتي أظهرت تأثيرات وقائية أكثر من المجموعتين الأخريين، ثم المجموعة التي تناولت الليدوكايين بمفرده، وأخيراً المجموعة التي تناولت المحلول الملحي العادي. كما أظهرت الدراسة أنه لا توجد فروق ذات دلالة إحصائية عند مستوى الدلالة (α≥0.05) في تأثير إستخدام دواء ديكساميثازون، ليدوكايين، وكلاهما معا، في منع التهاب الحلق والسعال وبحة الصوت لدى المريض الذي يخضع لعملية استئصال المرارة بالمنظار تعزى لمتغيرات: (العمر، والجنس، والوزن). لكن كانت هناك اختلافات تعزى لمتغير مدة العملية، حيث وجد أنه كلما طال الوقت كلما تعرض المريض لبحة في الصوت بعد العملية. وبناءً على نتائج الدراسة أوصت الباحثة باستخدام مزيج من الديكساميثازون والليدوكايين للوقاية من التهاب الحلق والسعال وبحة الصوت لدى المرضى الذين يخضعون لعملية استئصال المرارة بالمنظار والذي كان أكثر فعالية من الليدوكايين بمفرده والمحلول الملحي الطبيعي. علاوة على ذلك، هناك حاجة لدراسات أخرى بجرعات مختلفة من كل من الديكساميثازون والليدوكايين لدعم نتائج الدراسة الحالية.
Pre-operative anxiety correlation with post-operative pain level and analgesia consumption: A prospective cohort study

Introduction: In patients who are hospitalized for surgery, anxiety disorders are frequently observed. Anxiety affects the patient’s postoperative pain level and may significantly increase the consumption of analgesia post-operatively. Aim: The allover aim of this thesis is to identify and assess the relationship between preoperative anxiety and postoperative pain and its related factors. Also, to find the relationship between preoperative anxiety and post-operative Pethidine consumption among patients undergoing a laparoscopic cholecystectomy surgery . Method: A Prospective cohort design was used in this study. A convenience sample of total 100 patients were enrolled who were undergoing a laparoscopic cholecystectomy. Demographic characteristics of the patients were recorded. Beck’s Anxiety Inventory scale (BAIs) was administered to the patients. During the postoperative period, Pethidine was used for pain control. Visual Analog Scale (VAS) scores and Pethidine consumption of all patients were recorded. The setting of data collection was St. Joseph Hospital in Jerusalem district. The data was analyzed by SPSS (version 21). The result: Out of total sample, 35% of participants are male while 65% are female. Fifty-five percent had school education level and 45% had university level. Regarding ASA, most of participants (74%) were ASA1. Participants mean level of pain was higher in the pre-operative period (mean VAS= 1.3) comparing with their mean level of pain in the post-operative period (mean VAS= 0.5). According to BAIs, the most common feelings among the participants were psychological in nature. There is a statistical significant difference between the participant’s level of anxiety and post-operative pain level (p value < 0.001). Participants who had high level of pre-operative anxiety (BAIs ≥17) is tend to develop a higher level of post-operative pain level (VAS mean=7). Furthermore, there is a statistical significant difference between the participant’s level of anxiety and post-operative Pethidine consumption (p value < 0.001). Participants who had a high level of pre-operative anxiety (BAIs ≥17) were predisposed to consumed a higher dose of post-operative Pethidine (mean=1.1mg/kg). Gender, weight, level of education, and smoking were predictors of developing pre-operative anxiety among participants. Also, gender, smoking, and medication were statistically significant predictors for developing post-operative pain. Furthermore, gender, medical history, and medication were statistically significant predictors for Pethidine consumption post-operatively. Conclusion: Based on the analysis of the findings of the study, the following inferences were drawn. There was a significant relationship between preoperative anxiety with post-operative pain and post-operative pethidine consumption among laparoscopic cholecystectomy patients. Thus, the relationship between anxiety and pain was supported. Therefore, pre-operative anxiety reduction intervention should be promoted as an institutional policy and implemented as a routine care for all surgical patients.
Prevalence of Preoperative Anxiety and its Contributing Risk Factors in Adult Patients Undergoing Elective Surgery

Background: Hospitalization and waiting for a surgical operation are the main arising causes for patient anxiety preoperatively, which significantly affects the outcome of both anaesthesia and surgery. Yet, no researches have been conducted and there is currently no published data on the preoperative anxiety levels in the West Bank (Palestinian Territories). The purpose of this study is to assess the level of preoperative anxiety among patients waiting for elective surgery in Rafidia Governmental Hospital. Methods: A cross sectional study was carried out on 320 patients scheduled for elective surgery at the preoperative care unit (recovery room) in Rafidia Governmental Hospital, Nablus, West Bank (Palestinian Territories). The patients’ medical records were reviewed to obtain demographic data, the surgical and medical diagnosis, medication, and medical history of the study participants. All inpatients were assigned an ASA (American Society of Anaesthesiologists) physical status of I or II, were between 18 and 60 years old, and were able to give informed consent and answer questions were enrolled in this study. Illiterate patients, patients with psychological impairment, pregnant women, and alcoholic patients were excluded from the study. The Amsterdam Preoperative Anxiety and Information Scale (APAIS) was applied to assess the level of patient anxiety. SPSS was applied for statistical analysis and description. Results: The mean age ± SD of the sample was 37.20 ± 10.99 years. The majority of interviewed patients were females (197; 61.6%), married (239, 74.7%), and had no previous surgical operations (225, 70.3%). Less than one third of studied patients had a college education (101, 31.6%) and the majority were scheduled for general anaesthesia (267, 83.4%), while the remaining were scheduled for regional anaesthesia. Approximately one third of the sample ware smokers (108, 33.8%), and less than one half were scheduled for a major operation (137, 42.8%). The majority of patients were ASA category I (267, 83.4%). Using a cut-off point of 11, patients with an anxiety score ≥ 11 on the APAIS were considered anxious. Analysis showed that 184 (57.5%) patients were anxious. The results also indicated that 163 (50.9%) patients indicated a desire for more information about anaesthesia and surgery. Females were more anxious than males (p<0.001). Analysis also showed that non-smokers had significantly higher anxiety scores than smokers (p<0.001). There was no significant difference in anxiety scores between those who had college education and those with lesser levels of education (p=0.83). Patients with no previous surgery did not have significantly higher anxiety scores than those with surgical history (p=0.052). Patients with type II ASA had significantly higher anxiety scores than those with ASA type I (p=0.001). Patients scheduled for GA had significantly higher anxiety scores than those scheduled for regional anaesthesia (p=0.001). No significant difference was found between currently married and unmarried patients in anxiety scores (p=0.58). Patients over 40 years of age had no significant difference in anxiety scores compared with those less than 40 years of age (p=0.55). The results also showed that anxious patients had a higher desire for seeking information about anaesthesia and surgery than non-anxious patients. Conclusion: The prevalence of preoperative anxiety in the West Bank is high compared with results from developed countries, but it correlates with the results from developing countries. Female patients, non-smokers, patients with no previous history of surgery, ASA II patients, and patients undergoing surgery under general anaesthesia had significant anxiety levels. Our study showed that patients with high information requirements, females, and patients scheduled for general anaesthesia are more anxious preoperatively. Assessment of preoperative anxiety is an important step in developing new strategies for reducing anxiety. Assessment can be easily done with a questionnaire such as APAIS, which is a short and economical instrument, and its validity and reliability has been approved. Recommendations: This study advises that providing more information regarding surgery and anaesthesia might help in reducing preoperative anxiety levels. Also, it is a must to increase trust between patients and the medical team (doctors and nurses) by improving doctors’ and nurses capacity in building (doctor, nurse)-patient trust through training courses. This study was conducted in one governmental hospital and it is the first study conducted in the assessment of preoperative anxiety level, so further research should be conducted in other governmental and specialized hospitals in Palestine in this field to recognize and identify level and causes of preoperative anxiety in order to introduce effective methods and strategies to help reduce preoperative anxiety. This study revealed a high level of preoperative anxiety among surgical patients, which emphasizes on that ,new protocols and systems must be established and followed in our government hospitals to decrease the high level of preoperative anxiety levels, which leads decrease the intra and postoperative complications.
Prophylactic Ephedrine versus Phenylephrine for Preventing Maternal Hypotension in Women Undergoing Spinal Anesthesia for Cesarean Section

This study received an award from the conference for Scientific Excellence Background Hypotension during spinal anesthesia for cesarean section is secondary to the sympathetic blockade and aorto-caval compression by the uterus and it can be deleterious to both the fetus and the mother. Ephedrine and phenylephrine improve venous return after sympathetic blockade during the spinal block. Aims The aims of the present study are to compare the efficacy of ephedrine and phenylephrine in the prevention and treatment of maternal hypotension during spinal block, to evaluate the side effects of ephedrine and phenylephrine, and to assess fetal changes as measured by Apgar scores. Methods Fifty five women, American Society of anesthesiologist (ASA) Grade I and II, undergoing spinal anesthesia with Bupivacaine and Fentanyl for cesarean section were randomly divided into two groups to receive prophylactic ephedrine (n = 27, dose = 10 mg, i.v.) or Phenylephrine (n = 28, dose = 80 µg, i.v.) immediately at the time of providing the subarachnoid block. Mean (SD) age of Ephedrine group was 30.48 ± 5.5 vs. the Phenylephrine group, which was 31.64 ± 3.3. Hypotension was defined as a decrease in systolic arterial pressure of >20% from baseline values and was treated with bolus administration of the vasopressors at 50% of the initial dose. Maternal arterial pressure (BP) and heart rate (HR) were measured every 3 minutes by automated oscillometry. Ringer's lactate (RL) solution (20 ml/kg) was infused 30 minutes before spinal injection for all participants. Vital signs (blood pressure, heart rate, and arterial oxygen saturation) were recorded throughout the surgery. Maternal and neonatal perioperative complications were also controlled and recorded. The incidence of hypotension, reactive hypertension, bradycardia, tachycardia, nausea and vomiting, and Apgar scores on the 1st and 5th minutes were evaluated. Results There was an insignificant difference in demographic data between the groups. The mean (±SD) dose of ephedrine used was 19.81 mg (±5.46) and phenylephrine was 125.71 µg (±35.64). Changes in systolic and diastolic pressure were comparable in the two groups. There were significant differences in the incidence of reactive hypertension episodes (Ephedrine group: 48 (14.5%) vs. Phenylephrine group: 26 (7.7%) P < 0.005). There were no differences in the incidence of bradycardia ( Ephedrine group: 3(11.1%) vs. Phenylephrine group: 6 (21.4%) P > 0.301). There were significant differences in the incidence of nausea and vomiting (Ephedrine group: 10 (37%) vs. Phenylephrine group: 3 (10.7%); P> 0.018). There were no significant differences in the incidence of hypotension, with an incidence of 18(66.7%) in the Ephedrine group and 17(60.7%) (P <0.646) in the Phenylephrine group. Maternal arrhythmias were more common in the Ephedrine group at 10(37%) than in the Phenylephrine group at 7 (25%), but the difference is not significant (P=0.334).Additionally, maternal restlessness was more common in the Ephedrine group:8 (30.8%) than the Phenylephrine group: 3 (10.7%), but with an insignificant difference (P=0.068). Differences in the Apgar score in the 1st and 5th minute were not observed. Number of patients who required rescue dose in the Ephedrine group was 24 (88.9%), which was significantly higher than the Phenylephrine group at 20 (71.4%), P < 0.005). There are significant differences in the number of rescue doses of the two drugs. In the Phenylephrine group there was only one patient (3.6%) that had the rescue dose 3 times, and for the Ephedrine group there were 9 patients (33.3%) that had the rescue dose 3 times each, (P = 0.033). Conclusion We conclude from this study that phenylephrine 80μg has a similar vasopressor effect to that of ephedrine 10 mg for the prevention or treatment of maternal hypotension during spinal anesthesia for elective cesarean section, and that there is no difference in neonatal clinical outcomes as measured by the Apgar score. The applicability of the results is limited to healthy women with term fetuses. The clinical significance of bradycardia, reactive hypertension and intraoperative nausea and vomiting should not be overlooked. Giving Phenylephrine immediately at the time of providing the subarachnoid block is superior to ephedrine to reduce reactive hypertension, nausea, vomiting and requirements for vasopressors rescue medication. The results of this study support the use of phenylephrine for the maintenance of maternal arterial pressure during spinal anesthesia for elective cesarean section. Nurse Anesthetist Implications In view of maternal complications, the most important and noticeable complication was brief bradycardia (reflex bradycardia), which was transient and only occurred in a few cases (HR<60 per minute) that needed treatment with 0.5 mg intravenous Atropine based on policies and procedures for anesthesia clinic supervised by an anesthesiologist. Nausea and vomiting that responded rapidly to antiemetic medication was slightly high in the ephedrine group. None of the observed complications were serious enough to have a significant impact on either the mothers or newborns according to the Apgar score. The Relationship Between Body Mass Index and Difficult Tracheal Intubation Among Patients who have Elective Surgery Under General Anesthesia in Jenin Governmental Hospital – Palestine

Introduction: Difficult tracheal intubation is a common source of mortality and morbidity in many setting include: Operation, Intensive Care Unit (ICU), and Emergency ward. The incidence reported of difficult tracheal intubation is 0.1%–13% and reaches 14% in the obese population. The objective of the study was to investigate and compare the utility of Body Mass Index (BMI) as indicator of difficult tracheal intubations in patients who underwent for elective surgery under general anesthesia. Aim: To find out the relationship between BMI and difficult tracheal intubation among patients who went for elective surgery under general anesthesia. Background: The study was done by using the intubation difficulty scale (IDS), Mallampati score, and Wilson score, to confirm that obese patients are more difficult intubation than lean patients. Material and methods: Prospective observational descriptive design was used in this study to patients who underwent for elective surgery under general anesthesia. The sample was 100 patients. The study done in Jenin governmental hospital; the following information was obtained from participants for analysis: gender, age, height, weight, BMI, past medical history, past surgical history .Assessment Mallampati score, IDS, Wilson score, and the American Society of Anesthesiologists classification assigned by the anesthesiologist and senior doctors provider performing the endotracheal intubation. Result: The total number of IDS was (81%) of participants who underwent elective surgery under general anesthesia were categorized as slightly difficult intubation according to IDS score. And 2% of participants were classified as moderate to major degree of intubation difficulty. 17% of participants were labeled as easy degree of intubation difficulty. And there is correlation between the Mallampati class and Wilson score with BMI p < 0.001 and p =0.008 respectively. Conclusion and recommendation: In conclusion, data showed that BMI is a reliable indicator for difficult tracheal intubation in patients who underwent elective surgery under general anesthesia. Mallampati classification and Wilson score is a strong predictor of difficult tracheal intubation in patients who underwent elective surgery under general anesthesia in correlated to their BMI, age and gender are not predictor for difficult intubation according to IDS scale. Future prospective studies should aim to include controlled preoperative assessments and defined concept of difficult tracheal intubation
